Starting a New Non-Profit Organization

Required Documents:
Articles of Incorporation: Include Exempt Purpose Statement and Dissolution Statement as described in IRS Code section 501(c)(3).
Employer Identification Number (EIN).
By-laws of the Organization.
Minutes of Board Meetings.
Board Members Information: Names, addresses, and resumes.
Active Members Information: Names and addresses.
Inventory of Assets: Cash, furniture, equipment, property, pledges.
Inventory of Liabilities: Mortgages, accounts payable, loans.
Rent/Lease Agreements and Contracts.
Financial Statements: Revenue and expense statements for the last four years or as far back as available.
Written Reason for Formation and History of the Organization.
Mission Statement: For religious organizations, include Statement of Faith or Beliefs.
Activities, Operations, and Programs Documentation: Statement of purpose, food programs, fundraisers, promotional materials.
Financial Support Documentation: Sources of revenue including contributions, tithes, offerings, fundraisers.
